Febuxostat

Febuxostat is an oral xanthine oxidase inhibitor primarily used for the long-term management of hyperuricemia in patients with gout. Elevated uric acid levels in the blood can lead to deposition of urate crystals in joints and soft tissues, resulting in painful inflammatory attacks known as gout flares. Febuxostat helps control this underlying metabolic abnormality by reducing the production of uric acid, thereby lowering the risk of recurrent gout episodes and long-term joint damage.

The drug works by selectively inhibiting xanthine oxidase, the key enzyme responsible for converting hypoxanthine and xanthine into uric acid. By blocking this pathway, febuxostat effectively decreases serum uric acid concentrations. Unlike medications used for acute gout attacks, febuxostat is intended for chronic urate-lowering therapy and is typically started after an acute flare has been controlled. Consistent daily use is important to maintain target uric acid levels and prevent crystal deposition.

Febuxostat is usually administered once daily, with or without food, and dosing may be adjusted based on periodic uric acid monitoring. Patients are often advised that gout flares may temporarily increase when starting therapy due to mobilization of urate stores; therefore, prophylactic anti-inflammatory treatment may be recommended during the initial months.

Common side effects include mild liver enzyme elevation, nausea, rash, and joint discomfort. Liver function monitoring is generally advised during therapy. Febuxostat should be used cautiously in patients with certain cardiovascular risk factors, and therapy should always be guided by a healthcare professional. When appropriately prescribed and monitored, febuxostat is an effective option for sustained uric acid control and long-term gout management.
